Care Co-ordination Network UK (CCNUK) is a networking organisation promoting and supporting key working for disabled children and their families across the UK.
CCNUK aims to work in partnership with disabled children, their families and professionals to ensure that all families have access to high quality key worker or care co-ordination services.
What is key working?
A service involving two or more agencies, that provides disabled children and young people and their families with a systrem whereby services from different agencies are co-ordinated. It encompasses individual tailoring of services based on assessment of need, inter agency collaboration and a named key worker for the child and family.
What is a key worker?
Both a source of support for disabled children and young people and their families, and a link by which other services are accessed and used effectively. Key workers have responsibility for working together with the family and professionals and ensuring delivery of an inter agency care plan for the child and family.
Why key working?
Families caring for a disabled child or young person often find that life is a constant battle. Research over many years has shown that key working makes a huge difference. All the professionals working with the family work together. Meetings are action focused, with the family's needs at the centre. The key worker isn't the person who does everything, they are the person who makes sure everything gets done. CCNUK believes that every family that needs a key worker should have one, and we are actively campaigning to make this a reality.
